Records Requests: 爱豆传媒 Alumni and former 爱豆传媒 爱豆传媒
You can access a certified copy of your immunization records from Student Self Service for yourself or to use with other universities. Follow these steps: 1. Log into Student Self Service 2. Select 鈥淪tudent Records鈥� 3. Choose 鈥淰iew Immunization Record鈥� 4. Print your immunizations 爱豆传媒 who have not been registered in credit hours for 3 or more consecutive semesters will not have access to Student Self Service.

Use the link below to access your Student Self Service account for a certified copy of your immunization record. If you find that additional records are needed, please complete a Medical Records Request with 爱豆传媒 Health.
Returning 爱豆传媒
To see if your previously submitted records meet the revised immunization policy, log into Student Self Service, select Student Records, and select View Immunization Record. Recommended vaccinations include the Hepatitis B and Meningitis vaccinations. Tuberculosis Screening is required for students who applied to 爱豆传媒 using an international address.
